
INTELLECTUAL DISABILITY SERVICES

Case management functions as the door through which consumers must pass to access needed services. A total of 10 Case Managers provide assessment, linking/referral, and monitoring to approximately 400 individuals with intellectual disabilities within the three county areas. Both children and adults receive support through the ID program. Referrals come from many resources, including Social Services, schools, public health department, local hospitals, physician’s offices, families and friends. There is no waiting list for ID Case Management services and the program works diligently to provide a timely intake and assessment process. In the past several years the amount of children who receive supports in the ID program has grown significantly. Case Management supports, as well as all supports provided in the ID program are carried out using a person centered approach in hopes of assisting all individuals with intellectual disabilities to live a happy and satisfied life. Referrals for services are made to the ID Community Support Director or by contacting the ID Case Management office.
RESIDENTIAL SERVICES The Intellectual Disabilities Residential
Programs continue to grow. Cumberland Mountain has seven congregate
group homes serving 64 individuals. In addition, our non-congregate
residential services are now serving over 80 individuals in their homes.
Non-congregate services include Residential Support, Personal
Assistance, Respite, EPSDT services, DD Waiver, and ED/CD Waiver. Staff
encourages an atmosphere of self-expression, while maintaining dignity
and respect. Individuals and their families have increased residential
options and are provided the opportunity for increased consumer choice
and person-centered planning.
Baxter House
is a
twelve-bed residence in Buchanan County for adult males and females with
moderate to profound mental retardation. Staff provide 24-hour
supervision. Training is provided for personal skills, fine/gross motor
coordination, safety skills, social and communication skills, and
leisure activities. Involvement with family is encouraged. Families are
included in periodic planning for resident needs. This program is funded
by Medicaid as an intermediate care facility.
Cumberland Mountain Home is a fifteen-bed program in Tazewell County for adult males and females with moderate to mild mental retardation. Staff provide 24-hour supervision. Residents of Cumberland Mountain Home are generally more independent in their functioning than residents of Intensive Care Facilities and require assistance rather than training for all areas of independent living. Emphasis is placed on community integration. Residents and family take part in service planning. Payment for room and board is made by the resident and is usually a percentage of the SSI or Social Security benefit. Generally, services are billed under Medicaid Waiver. Lebanon Group Home
is a eight-bed residence for
adult males who are transitioning into independent living. Residents
must demonstrate the ability to perform daily activities of living
independently and must also demonstrate safety at home and in the
community. Residents play an active role in planning the leisure
activities, which may include unsupervised activities in the community.
Residents and families are active in service planning. Payment for room
and board is made by the resident and is usually a percentage of the SSI
or Social Security benefit. Generally, services are billed under
Medicaid Waiver.
Residential Support Services
(In-Home Services)
is a
non-congregate service providing
training and support in the individual’s home or licensed residence.
This service may be needed by an individual with challenging behavior.
Emphasis is on a person-centered approach which lends to the development
of the lifestyle chosen by the individual. Residential Support Services
has enabled these individuals to maintain or improve their community
living arrangement which has prevented the need for placement in a
facility.
Personal Assistance Services
is a non-congregate service
provided for individuals who need in-home support in order to remain in
the family home. For these individuals, training and skills development
under Residential Support Services is not appropriate. This service is
equivalent to the personal care provided by the health department.
Assistance is provided in personal care, activities of daily living, use
of community resources, and medication/health related needs. Respite Care provides temporary, substitute care normally provided by the family or other caregivers. Respite care can be provided in the consumers home, the respite providers home, or in a community respite setting. This service is also provided on an emergency basis when caregivers are unexpectedly hospitalized or otherwise unable to provide normal care.
The Early Intervention Program serves
children birth through two years of age who have a diagnosed disabling
condition, a developmental delay or atypical development. The program
provides supports and services that focus on increasing the child’s
participation in family and community activities that are important to
the family. In addition,
supports and services focus on helping parents and other caregivers know
how to find ways to help the child learn during everyday activities.
Parents are asked to be full participants in planning and
carrying out the services their child receives. Services provided are
based on the
individualized outcomes desired for the child, and are available to all
eligible children and their families regardless of the family’s ability
to pay. Parents teach their children everyday – they are
the ones who make the most difference in their child’s development. The Early
Intervention Program is a service provider for the Infant & Toddler
Connection of Cumberland Mountain.
CUMBERLAND INDUSTRIES / DAYSUPPORT PROGRAM
Historically,
Cumberland Industries
was developed primarily to offer vocational and pre-vocational
opportunities to individuals with intellectual disabilities in our three
county areas. Many had the opportunity to work at various jobs both
within the sheltered workshop program as well as in the community. With
the many issues however that have occurred in our economy, as well as
funding changes that have come about in our system, it became necessary
for the program to make changes to better accommodate the individuals
that we provide supports for. Cumberland Industries is now primarily a
day support program that offers individuals with intellectual
disabilities the opportunity to work on areas that they feel will make
their lives happier and more satisfying. There is a primary focus on
developing appropriate supports using a person centered approach that
puts emphasis on what the individual feels that they need and would like
to improve in their life. Individualized support plans are developed for
each individual upon entry into the program to assist them in further
developing skills and opportunities that they desire to improve on or
explore. Some areas that are often focused on are
social, personal living, domestic and community living skills.
Frequently community outings occur that provide opportunities for more
community integration and socialization with others. There also continue
to be some opportunities for individuals to work in programs where they
can learn work skills and receive wages. The main Cumberland Industries
program is located adjacent to Southwest Virginia Community College. A
second location that provides solely a day support program is located in
Buchanan County, in the Keen Mt. area. Every effort is made by
Cumberland Mt. Community Services to provide or help arrange
transportation to these programs. More information about the programs,

Summer Program-
In addition to the options that are offered at both locations, a summer
youth program is also a major part of the day support program. Children
with intellectual disabilities from the three county areas are provided
with a program during the summer months that is designed to give them
the opportunity to participate in a wide variety of outings. The program
is primarily designed to provide supports that will enhance skills and
will help to make the child’s life happy and satisfying. The program
accomplishes this by providing many different types of outings and group
activities. Through the program the child meets old and new friends, and
also begins some preparation for the adult day support program if they
later decide to participate. Some examples of activities that are
offered are trips to Gatlinburg, minor league baseball games, fishing
trips, visits to Tweetsie Railroad, bowling, movies, picnics and many
more. More information about this program can be obtained by the

Cumberland Mountain also continues to provide a catering program that is located within the main Cumberland Industries building. Employment opportunities are provided in the catering program to individuals receiving services in both the ID and MH programs. Catering jobs are provided to area businesses, groups and people within the area. A new and exciting project was taken on last year with the development of Cumberland Café. The Café offers breakfast, a great salad and food bar and a nice lunch menu.
